HI - I'd like to be able to set up a form for interviewers to fill in when interviewing a candidate. I know you can do questions and answers but we have checkboxes we use for ratings and can't include these in the current set up.

We have a feature called "interview templates" designed to do exactly this, and they include the ability to add ratings. You can create them from the admin panel here >>
